Living Well with Chronic Conditions
Formerly the Chronic Disease Self Management Prograam, CDSMP, shows strong evidence of beneficial effects on physical & emotional outcomes, and health‐related quality of life for attendees who participate in these workshops. Central MN Council on Aging It is a fact that 87% of older adults have at least one chronic condition, and 67% have multiple conditions. CDSMP workshops offer older adults the opportunity to learn the skills to coordinate all the things needed to manage their health, as well as to help them keep active in their lives, manage their disease, and improve their health related quality of life. The program consists of a series of six workshops offered in community settings such as senior centers, churches and hospitals and is not disease-specific. The workshops are facilitated by a team of two trained leaders, pairing either a health professional with one or two community lay leader volunteers who have a chronic disease. It is the process in which the program is taught that makes it effective. Classes are highly participative, where mutual support and success build the participants’ confidence in their ability to manage their health and maintain active and fulfilling lives. Funded though a partnership of the MN Board on Aging and the MN Dept of Health under a grant from the Administration on Aging, this program has been sucessfully implemented in over 20 sites, through diverse setting such as clinics, senior centers, community centers, churches and senior housing. |
