Eat Better Move More is an outcome-based chronic disease management program for seniors, designed to help them manage their chronic health issues and prevent falls in order to maintain their independence.

The cost of health care and nursing home stays are three to five times more costly for seniors 65 and older. These higher costs are primarily the result of chronic health issues that are caused by modifiable behaviors in diet, exercise and lifestyle choices. By offering regular opportunities for education about managing chronic health issues, nutrition education, lifestyle and medication management as well as weekly exercise and walking club classes, seniors are able to improve their over-all health and fitness.
The Eat Better Move More program is offered in community settings such as nutrition sites and senior centers to make the program accessible and improve the ability of seniors to incorporate these lifestyle changes into their daily routine for long term improvement. The program continues year round with regular change-ups in educational topics, and exercise routines to help maintain interest and involvement.
The exercise classes address balance, flexibility, strength training and cardiovascular endurance. The program combines traditional exercises, Tai Chi and walking to better ensure fall prevention. Outcomes are continuously tracked through participant surveys and standardized fitness assessments. The program is also operated at culturally diverse sites.
