Live Well At Home – Minnesota’s Nursing Home Diversion Strategy

Minnesota’s diversion support model identifies and diverts high-risk, private pay older adults up-stream from Medicaid eligibility, from permanent nursing home use and spend down to Medicaid.

The target population for diversion support services is older adults, at risk of moving out of their home/apartment, and not eligible for publicly-funded Medicaid, Waiver or state programs. Partners and/or AAA staff conduct a “rapid screen” to assess for risk of nursing home or other placement. The rapid screen is a pilot-tested, 7-question risk screen tool applied at critical access points. The Live Well At Home model is a person-centered approach which blends coaching, risk management, and self-directed service models.
The program seeks to empower high-risk persons to self-manage risks with evidence-based health and chronic disease management care, caregiver support programs and self-directed services.

Diversion Support tools include educating consumers on addressing and reducing risk in specialized consultation and follow up provided by Care Consultation staff. Consumer directed support is also facilitated with the consumer to be used for purchase of self-directed community supports that meet needs and also address the reduction of risk to independence.

Funded through a partnership of the MN Board on Aging under a grant from the Administration on Aging, this program has been successfully implemented with over 25 individuals/families; 12 persons using a consumer directed model.
